راهنمای پذیرش و ترخیص

فرآیند پذیرش بیماران الکتیو

1- خوش‌آمد گویی و همراهی بیمار توسط واحد تکریم و هدایت بیمار به سمت اتاق IPD

2- پذیرایی از بیمار در واحد IPD و انجام امور پذیرش و بستری(پذيرش الكتيو)

3- بررسی دستور بستری بيمار و توجيه بيمار از نظر قيمت و هزينه شب تخت ها توسط کارشناس IPD

4- هدايت بيمار به اورژانس جهت اخذ شرح حال توسط پزشك اورژانس

5- برگشت بيمار به دفتر IPD جهت تشكيل پرونده

6- ثبت اطلاعات هويتی بيمار و همراه بيمار در قسمت پذيرش سامانه جامع

7- چاپ پرونده و اخذ مبلغ به عنوان علی‌الحساب و اعلام رضایت بیمار جهت دریافت خدمات درمانی

8- چاپ دستبند بيمار و بستن دور دست بيمار

9- تحويل يك عدد كارت همراه به بيمار و تحويل پرونده به نيروd تكريم و هدايت بيمار به بخش

Elective patient admission process

Welcoming and accompanying the patient by the reverence unit and directing the patient to the IPD room

Admitting patients in the IPD unit and performing admission and hospitalization procedure (elective admission)

Reviewing the patient’s admission order and clarifying the prices and hoteling costs to the patient by the IPD expert

Directing the patient to the emergency room to record a medical history by the emergency doctor

Returning the patient to the IPD office to file a case

Registering the patient’s and companion’s identity information to the comprehensive system in the admission section

Printing the file, collecting the deposit, and declaring the patient’s consent to receive medical services

Printing the patient’s wristband and tying it around the patient’s hand

Handing over a card to the patient, handing over the file to the reverence team, and directing the patient to the ward

پذیرش سرپایی اورژانس

1- اطلاع به کارشناس مسئول IPD توسط پرستار IPD

2- ترياژ بيمار توسط پرستار IPD

3- تعيين سطح ترياژ توسط پرستار

4- تكميل برگه ليست اقدامات درخواستي مثل ويزيت پزشك IPD و … توسط پرستار و ارائه به بيمار جهت تحويل به پذيرش براي ثبت و اخذ وجه

5- ارجاع بيمار به اتاق پزشك IPD

6- ثبت درخواست اقدام بعدي مثل انفوزيون يا سونداژ توسط پزشك براي تحويل به پذيرش براي دريافت وجه

Emergency outpatient admission

Notifying the IPD expert by the IPD nurse

Performing patient triage by IPD nurse

Determining the triage level by the nurse

Completing the list of requested measures, such as IPD doctor’s visit and… by the nurse and presenting it to the patient to be handed over to the reception for registration and payment

Referring the patient to the IPD doctor’s room

Registering the request for further procedures, such as infusion or sounding by the doctor to be submitted to the reception for payment

پذیرش تحت نظر اورژانس

1- اطلاع به کارشناس مسئول IPD توسط پرستار IPD

2- ترياژ بيمار توسط پرستار IPD

3- تعيين سطح ترياژ توسط پرستار

4- تشكيل پرونده تحت نظر توسط نيروی پذيرش به درخواست پزشک طب اورژانس

5- چاپ اوراق پرونده و اعلام رضایت بیمار جهت دریافت خدمات درمانی

6- چاپ دستبند بيمار تحويل پرونده به منشی بخش جهت ثبت اقدام

7- درصورت پايدار شدن حال بيمار، توجيه همراهی برای هزينه ها و اخذ مبلغ علی الحساب

فرآيند ترخیص بيمار بستری در بخش

1- اعلام ترخيص بيمار توسط پزشك معالج

2- تکمیل پرونده بیمار توسط پزشک معالج و پزشک IPD

3- تدوین خلاصه پرونده بیمار به زبان انگلیسی با کلیه اصطلاحات مورد نیاز و مطابق دستورالعمل توسط پزشک IPD با مشورت پزشک معالج

4- اطلاع به کارشناس IPD در زمینه ترخیص بیمار توسط پرستار IPD

5- ارائه آموزش های لازم قبل از ترخیص و اطلاعات مورد نیاز از قبیل پیگیری روند درمان توسط پزشک و پرستار IPD

6- ارائه کلیه اطلاعات مورد نیاز در زمینه راههای برقراری ارتباط با بیمارستان (ایمیل، تماس تلفنی، سایت) در فرم های طراحی شده توسط کارشناس IPD

7- انجام اقدامات لازم جهت ترخیص بیمار توسط کارشناس IPD و دریافت مبلغ مورد نیاز مطابق صورتحساب تدوین شده

8- ارائه فرم خلاصه پرونده به زبان انگلیسی به بیمار

Discharge process of a patient hospitalized in the ward

Announcing the discharge of the patient by the attending physician

Completing the patient’s file by the attending physician and the IPD physician

Compiling the summary of the patient’s case in English with all the required terms and according to the IPD physician’s instructions in consultation with the attending physician

Informing the IPD expert of the patient’s discharge by the IPD nurse

Providing necessary instructions and required information before discharge, such as following up the treatment process, by the IPD doctor and nurse

Providing all the required information regarding the means of communication with the hospital (email, phone call, website) in the forms designed by the IPD expert

Taking the required measures to discharge the patient by the IPD expert and receiving the due amount according to the prepared invoice

Providing the case summary form, in English, to the patient
